Required Consent Forms - SB12
DVISD Families,
Recent changes in state law mean schools must now have parental permission for certain student services and activities. These services may previously have been offered without a requirement for parent consent. Del Valle ISD wants every family to feel informed and involved in their child’s education.

Why Are Consent Forms Needed?
A new Texas law requires schools to have written parent consent before students can receive some counseling services, medical care from the campus nurse, or join school clubs and organizations. This law took effect after registration, meaning these forms were not included in our registration process this school year.
New: Services That Require Consent
Health Services: A parent must provide consent for the health-related services listed below. This consent does not take the place of an Individualized Health Plan (IHP), Section 504 Plan or individualized education plan (IEP).

First aid and injury evaluation

Administration of over-the-counter medications in accordance with law

Administration of prescription medications in accordance with law

Monitoring of chronic health conditions (e.g., asthma, diabetes, seizures)

Vision, hearing, and scoliosis screenings
-
Under the influence assessment
-
Lice screening
-
Acanthosis screening
-
Support during illness or physical symptoms at school
-
Coordination of health services
-
Heat illness prevention and injury support for student athletes
-
Referrals to Community Resources
-
Mental Health Support
-
Promote Health Education (e.g. nutrition and hygiene)

Immunization Compliance
These services are designed to keep students safe and ready to learn. If you choose to opt out and your child has a non-life-threatening illness or injury at school, the nurse cannot provide care and will contact you. The campus nurse and staff will be able to provide immediate care for situations determined to be an emergency.
Counseling Services: We also need your consent to provide for school-based counseling services. This includes mental health support. These services are offered by trained staff, and your consent will remain in effect for the current school year unless you choose to rescind it earlier.
Counseling care that requires consent includes:

Psychological or psychiatric examinations, tests, or screenings

Psychological or psychiatric treatment designed to change behavior, emotions, or attitude.
A parent may not opt out of the following:

School district emergency responses;
-
Law enforcement or Department of Family and Protective Services activities;

Behavioral threat assessment required by law; or
-
Other rights or duties required by law, including the Texas Family Code.
Participation in clubs or organizations: By law, students cannot join school-sponsored clubs, including Career and Technical Student Organizations (e.g., DECA, HOSA, Robotics), without written parent permission. All clubs and organizations will provide permission forms to students that must be completed before participation.
How to Complete the Forms
All forms are available through Skyward Family Access. Here’s how:

Log in to your Skyward Family Access account: www.dvisd.net/skyward

Click on the “Online Forms” tab.

Open the Parental Consent Forms section.

Review the information carefully, then mark your consent choices.
-
Submit your completed forms.

If you need help logging in or completing the forms, call your school’s front office.
What If I Don’t Give Consent?

Medical services: The campus nurse won’t provide non-emergency medical care to your child, but will still act in an emergency.

Counseling services: Your child won’t be able to receive certain counseling services offered at school, but will still respond to emergencies as allowed by law.

Clubs/organizations: Your child will not be able to participate until a signed form is on file.
