Notice of Availability – Form 1095-C
Pursuant to the Paperwork Burden Reduction Act (H.R. 3797) enacted on December 23, 2024, form 1095-C will no longer be automatically mailed to employees. Instead, form 1095-C for the 2025 calendar year is available upon request.
Form 1095-C (Employer Provided Health Insurance Offer and Coverage) provides information about health insurance coverage offered to you through Del Valle ISD.
This form is not required to file your federal income tax return, but you may keep it for your records.
How to Request Your Form 1095-C
Employees may request a copy of their form 1095-C through ETC (Eligibility Tracking Calculators), the
district’s third-party administrator for ACA reporting.
To request your form, please visit:

1. Click “ETC RETRIEVE” in the menu bar
2. Complete the request form and submit
3. Your Form 1095-C will be delivered via email from:
1095-retrieve@eligibilitytrackingcalculators.com
Once requested, the form will be provided within 30 days, in accordance with IRS guidelines.
This notice will remain posted through October 15, 2026.

WHEN YOU ARE ELIGIBLE
You are eligible for coverage if you are an active employee and working 20 hours or more per week and are actively contributing to the Teachers Retirement System of Texas.
You have 31 days from your actively at work date (start date) to complete your paperwork and submit to the Benefits Coordinator. Participation in the health-care benefits will be effective on either (1) employee’s first active day of employment (physical start date) or (2) the first of the month following the first active day of employment (physical start date).
Should a new employee choose not to enroll within the initial 31 day sign-up, the next opportunity for enrollment will be during the next Annual Enrollment. (Certain special events such as a change in marital status, dependent status or employment status of an employee or spouse, may qualify for an exception to this rule. Contact the Benefits coordinator within 31 days of the qualifying event for further clarification.)
SUPPLEMENTAL BENEFITS
If you are an active employee and working 20 hours or more per week, you are eligible to enroll in our supplemental benefits program on the first day of the month after your start date.
You have 31 days from your actively at work date (start date) to complete your benefits enrollment online at www.dvisd.bswift.com.
